Print off your own prescription form~
Prescription items can only be delivered once you have obtained a prescription from your vet and sent this to us. Your vet must legally write you a prescription for free so long as your animal requires the medication. Once you have the prescription, you can scan/email, fax or post it to us, click here for info.
At the time of getting a prescription, you should ask your vet to make it a repeat prescription and this will save you having to go back to your vet each time you require more medication. A prescription can be valid for up to 6 months allowing you to buy the same medications for less from us.
Your veterinary surgeon will have a prescription form to use or alternatively, you may find it easier to complete our blank prescription form below and ask your vet to complete this on your behalf.
